My narc ex has spent the last 4 years playing me and 2 other women .
Having relations between the 3 of us with many lies and deceat . He got caught out in September by 2 of us and it came out he gave us both h.p.v .
He is now with the one (who is most vulnerable) who knows absolutely nothing of how he's been playing us for years or h.p.v . Do I tell her she's being played or keep her in the dark ?

she won't believe you.
Apart from that, it is not your business. She'll have to find out herself. Or hear it from multiple sources she trusts and are objective.
