Unfortunately 7 Cups does not allow surveys. This is part of rule 9 in the forum guidelines. And your survey form invites people to provide contact details, which is not allowed either. So for those reasons I think it is likely that someone will remove the link from your post.
There are other forums on the Internet where you probably can invite people to take part in surveys.

@wakingPhoenix hello there! i am really glad to hear that you were able to overcame the depression and i can only imagine how tough you must be! Unfortunately, as @RarelyCharlie mentioned below, creating surveys here on 7cups is actually against the guidelines, so you may want to do your survey on another website.
