You are transgender when your biological sex (female / male) does not match with how you identify yourself as. For example, you may have a vagina but you don't identify as a woman, but rather as a man. The same can happen for a person who has a penis but identifies as a woman. Gender is our self-identification as a man or a woman (or none, or both even), and does not depend on our biological sex.

If you identify yourself as the gender you were not assigned with at birth. So if your physical form is female, but you mentally and emotionally feel like you're male or the opposite. And you feel trapped in your own body perhaps, because your outer appreance doesn't reflect how you believe and feel you are on the inside.

To answer this, you should know the difference between sex and gender. Your genitals determine your sex, your brain determines your gender. People whose sex and gender do not match up are transgender. For example, if you were biologically born with female genitals but you do not identify as a girl and felt like you were a boy, you are transgender.

At the heart of the question "How do I know if I am transgender?" is usually someone feeling misaligned with their assigned gender at birth. This misalignment is discomfort in your current gender and your current gender role.
