Located in Richardson, TX, CardiacFITT is a dedicated Healthcare Management program focused on enhancing the health and quality of life for individuals grappling with two or more chronic health conditions. This physician-directed and nurse case-managed initiative offers a tailored approach that allows participants to actively engage in their own healthcare journey. Through a comprehensive suite of services, including registered nurse visits, health education, nutrition guidance, exercise instruction, chronic health coaching, registered dietitian support, and care management, CardiacFITT equips individuals with the necessary tools and resources to effectively manage their health. We believe that accessible care is essential; therefore, our services are offered on a sliding scale, ensuring that no one is denied assistance due to financial constraints.
