Nutrition for Longevity (N4L) in Fort Worth, TX, is dedicated to enhancing health and well-being through our Home Delivered Meals and Medically Tailored Meals program. We cater to eligible clients covered under Medicare Advantage plans, Medicaid Long-Term Services and Supports, or the Older Americans Act, as well as offering a self-pay option for individuals wanting nutritious meals delivered right to their door. Our meal plans, developed by a Registered Dietitian, are designed to meet specific dietary needs and accommodate various medical conditions such as Chronic Kidney Disease and Congestive Heart Failure. We prioritize health by providing meals that are free from dairy, peanuts, gluten, and pork. For those receiving Medicaid or Older American Act benefits, meal eligibility varies based on state and program, and Case Managers can assist with determining service availability. Medicare Advantage beneficiaries should check their plan materials for meal coverage details. Additionally, for individuals not qualifying for assistance, our self-pay program offers customizable meals at an affordable rate of $7.75 each. N4L also provides Medical Nutrition Therapy and Nutrition Counseling services, ensuring comprehensive nutritional support for everyone in our community.
