Living with Type 2 Diabetes Program

The Living with Type 2 Diabetes Program in Smyrna, TN, is dedicated to supporting individuals recently diagnosed with type 2 diabetes through a comprehensive and accessible learning experience. Over the course of 12 months, participants engage in a free program designed to empower them with knowledge about diabetes management. Offered in both English and Spanish, this program includes six informative e-booklets tailored to address various topics, as well as a monthly e-newsletter filled with seasonal tips and local community resources. Enrollees who provide their phone numbers are invited to join the monthly 'Ask the Experts' Q&A series, where they can pose questions to diabetes specialists, learn about the connection between diabetes and heart health, and enjoy exclusive giveaways aimed at enhancing their well-being.
