OPTIONS for Community Living is a vital program located in Hermitage, TN, dedicated to supporting the elderly and adults with disabilities who may not qualify for the CHOICES program. Individuals can easily determine eligibility through our Intake Specialist using a straightforward Intake Form, leading to an in-home assessment by our caring case management staff.
