Located in New Kensington, PA, Cancer Transitions is a supportive, evidence-based program dedicated to helping cancer survivors navigating life after treatment. Designed for individuals who have completed their cancer treatment within the last 24 months, our program offers vital resources including nutritional support, tailored exercise plans, emotional health education, and medical management guidance. While primarily focused on recent survivors, we welcome all individuals at any stage of their survivorship journey. Our mission is to empower cancer survivors with the tools and knowledge necessary to enhance their quality of life and foster a path to holistic wellness.
