Located in Mckeesport, PA, Cancer Transitions is a dedicated program designed to support cancer survivors who have completed their treatment, focusing particularly on those who have finished within the last 24 months, though our resources are beneficial at any stage of survivorship. Our mission is to empower individuals through comprehensive support that includes nutritional guidance, personalized exercise plans, emotional health education, and medical management assistance.
