Cancer Transitions is a compassionate program located in Lititz, PA, dedicated to supporting cancer survivors who have recently completed their treatment journey. Tailored for those within 24 months post-treatment, this evidence-based program recognizes that assistance can be beneficial at any stage of survivorship. We offer comprehensive resources including nutritional support to promote healthy eating, customized exercise plans to enhance physical well-being, emotional health education, and key medical management information—all aimed at fostering resilience and empowering individuals to thrive after cancer. Our mission is to provide a caring environment where survivors can find the guidance and tools they need to navigate life beyond a cancer diagnosis.
