Cancer Transitions is a compassionate program based in Greensburg, PA, dedicated to supporting cancer survivors who have recently completed treatment. Designed for individuals within 24 months of finishing their care, the program is available to anyone at any stage of their survivorship. Our evidence-based approach offers vital resources including nutritional support, customized exercise plans, emotional health education, and comprehensive medical management guidance.
