Located in Ephrata, PA, Cancer Transitions is a dedicated program designed to empower cancer survivors who have completed treatment, particularly those within the past 24 months. We understand that the journey does not end with treatment, and our program is here to support survivors at any stage in their recovery. Through evidence-based strategies, we offer essential services including nutritional support, personalized exercise plans, emotional health education, and comprehensive medical management education.
