Health and Wellness Services - Project HEALTH

Located in Chester, PA, Project HEALTH, an initiative of Horizon House, is dedicated to providing comprehensive health and wellness services that empower individuals to make informed choices about their health. We offer a supportive environment with resources including a wellness room, peer support, and assistance in navigating the healthcare system. Our program provides both group and individual sessions, nutritional counseling, tobacco recovery programs, and exercise and fitness activities to encourage a holistic approach to well-being. We welcome clients with Medicare and Medicaid, ensuring that everyone has access to essential health services without barriers.
