Blended Service Coordination Program

The Blended Service Coordination Program, located in Allison Park, PA, is dedicated to empowering individuals with primary mental health conditions—children, adolescents, and adults alike. Our mission is to provide comprehensive support that enables personal growth and self-direction through tailored services including education, advocacy, and recovery planning. By fostering networks and facilitating problem resolution, we help individuals navigate their paths to recovery and health management. Our program offers two levels of care to meet varying needs: Level one provides weekly contact for more intensive support, while Level two includes monthly check-ins. We are committed to ensuring our clients receive the necessary guidance during regular business hours and have access to 24/7 on-call emergency services. A referral is required for enrollment, and we are here to assist in navigating the recovery journey with care and expertise.
