Our comprehensive Home and Community Based ADvantage Waiver program offers a range of essential services designed to foster autonomy and improve quality of life. These services include case management, personal care, skilled nursing, home-delivered meals, specialized medical equipment, prescription medications, home modifications, assisted living, adult day health, and hospice care.
