ADvantage is dedicated to empowering eligible adults and individuals with disabilities in Pryor, OK, to maintain their independence and remain connected to loved ones through our Home and Community Based Waiver program. We offer vital services that include personalized case management, skilled nursing, essential home-delivered meals, and access to specialized medical equipment. In addition, our program supports prescription medication needs, home modifications for enhanced living environments, assisted living arrangements, adult day health services, and compassionate hospice care. We believe in providing a supportive pathway that allows individuals to thrive in their own homes. Applications for our program can be conveniently completed online, or those needing assistance can schedule a phone application through the Medicaid Services Unit.
