Nutrition for Longevity (N4L) is dedicated to enhancing the health and well-being of residents in Westlake, OH, by providing Home Delivered Meals and Medically Tailored Meals to those eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act. Our program is designed with the help of Registered Dietitians to deliver nutritious meals that cater to a wide range of medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, while ensuring they are free from dairy, peanuts, gluten, and pork. We work closely with case managers and local agencies to assist Medicaid and Older Americans Act recipients in navigating their eligibility for our meal delivery services. For Medicare Advantage beneficiaries, we encourage checking plan materials to find meal inclusion in their benefits. Additionally, we provide a self-pay program for those who seek the convenience of home-delivered meals at an affordable rate of $7.75 per meal. We also offer Medical Nutrition Therapy and Nutrition Counseling, confirming coverage through individual insurance plans for added support on the journey to better health.
