PASSPORT Program

The PASSPORT Program in Tiffin, OH, is dedicated to empowering frail and low-income older adults to maintain their independence within the comfort of their own homes. Part of the Area Office on Aging, our consumer-directed service options allow individuals aged 60 and over to take charge as employers of record, selecting their own caregivers from friends, neighbors, and some family members. We provide essential services, including meal delivery, housekeeping, personal care assistance, transportation, emergency response systems, and access to home medical equipment and supplies. Additionally, our program offers adult day services and minor home modifications to enhance the living conditions of our clients. We are committed to supporting those eligible for Medicaid's Assisted Living Waiver, ensuring that every older adult receives the care they deserve while fostering their autonomy and dignity.
