PASSPORT Program

The PASSPORT Program, located in Sylvania, OH, is dedicated to supporting frail and low-income older adults in maintaining their independence and dignity by enabling them to live comfortably in their own homes. Our consumer-directed service options empower individuals aged 60 and over to take charge as employers of record, allowing them to select their own service providers, including friends, neighbors, and some family members. We offer a range of essential services including meal delivery, housekeeping, personal care assistance, transportation, emergency response systems, home medical equipment, adult day services, and minor home modifications. Designed for those eligible for Medicaid's Assisted Living Waiver, the PASSPORT Program is committed to enhancing the quality of life for seniors in our community.
