PASSPORT Program

The PASSPORT Program in Sandusky, OH, empowers frail and low-income older adults to maintain their independence while remaining in the comfort of their own homes. As part of the Area Office on Aging, we offer consumer-directed service options that enable individuals aged 60 and over to take control of their care by choosing their own service providers, including friends and family. Our comprehensive range of services includes meal delivery, housekeeping, personal care assistance, transportation, emergency response systems, and adult day services. We also provide access to essential home medical equipment and supplies, and minor home modifications to ensure a safe living environment. Designed for those eligible for Medicaid's Assisted Living Waiver, the PASSPORT Program is dedicated to supporting the well-being and dignity of our older adults in the community.
