Ohio Home Care Program

The Ohio Home Care Program, based in Sandusky, OH, is dedicated to enhancing the quality of life for individuals with chronic conditions and disabilities by offering essential services that allow them to remain in their homes and communities. Through CareStar's comprehensive assessment and case management services, we facilitate access to a wide range of support including skilled nursing care, personal care, homemaking assistance, and home-delivered meals. Our program also provides vital aid such as personal emergency response systems, accessible home modifications, and minor home maintenance services. We strive to ease the burdens of daily living by offering specialized medical equipment, transportation assistance, and respite care, ensuring that our clients receive the compassionate, tailored service they need to thrive in a familiar environment. As part of the Ohio Department of Medicaid’s waiver services, we work diligently across all 88 counties to create a supportive network for those we serve.
