Located in Painesville, Ohio, the Ohio Home Care Program, operated by CareStar, is dedicated to providing vital assessment and case management services across all 88 counties in Ohio. Our mission is to empower individuals with chronic conditions and disabilities to receive comprehensive care in the comfort of their own homes, rather than in institutional settings. We proudly offer a range of essential services including skilled nursing care, personal care, homemaking assistance, home-delivered meals, personal emergency response systems, accessible home modifications, and home maintenance for minor repairs. Additionally, our program provides specialized medical equipment, vehicle modifications, out-of-home respite care, adult day care, and supplemental transportation. Through the Ohio Home Care Waiver Program, we aim to promote independence and enhance the quality of life for those we serve, ensuring they have the support they need within their communities.
