Ohio Home Care Program

The Ohio Home Care Program, based in North Olmsted, OH, is dedicated to empowering individuals with chronic conditions and disabilities by providing essential home care services. Our comprehensive approach includes skilled nursing care, personal assistance, homemaking support, and home-delivered meals to ensure that everyone can receive the care they need in the comfort of their own homes. We also offer personal emergency response systems, accessible home modifications, minor home repair services, specialized medical equipment, and out-of-home respite care, along with adult day care and supplemental transportation to enhance independence and quality of life. As part of the Ohio Department of Medicaid’s waiver services, our mission is to facilitate access to critical support, allowing individuals to thrive in their communities rather than in institutional settings.
