Ohio Home Care Program

The Ohio Home Care Program, based in Mount Vernon, is dedicated to empowering individuals with chronic conditions and disabilities by providing essential in-home care services throughout all 88 counties of Ohio. Our mission is to help individuals lead independent lives within their own communities by offering a comprehensive range of support including skilled nursing care, personal care, homemaking assistance, and home-delivered meals. Additionally, we facilitate access to personal emergency response systems, home modifications, minor home repairs, specialized medical equipment, respite care, adult day care, and supplemental transportation. Through the Ohio Home Care Waiver Program, we strive to ensure that eligible individuals can remain in the comfort of their homes rather than facing the challenges of long-term care facilities or hospitals, reflecting our commitment to compassionate and practical service.
