Located in Marietta, OH, Nutrition for Longevity (N4L) specializes in providing essential Home Delivered Meals and Medically Tailored Meals for individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports, or the Older Americans Act. Our dedicated program gives priority to clients' nutritional needs, with meals crafted by a Registered Dietitian that cater to a range of medical conditions, including Chronic Kidney Disease and Congestive Heart Failure. We also accommodate dietary restrictions by offering meals that are free from dairy, peanuts, gluten, and pork. For those eligible through Medicaid or the Older Americans Act, please consult with your Case Manager for meal delivery benefits; alternatively, reach out to your Medicaid provider or local Area Agency on Aging for assistance. Medicare Advantage beneficiaries may find meal options included in specific post-discharge or chronic care support programs. For individuals without qualifying health plans, our self-pay program allows you to choose every meal at an affordable rate of $7.75 each, with a flat rate shipping fee.
