Ohio Home Care Program

Located in Mansfield, Ohio, the Ohio Home Care Program is dedicated to providing compassionate assessment and case management services across all 88 counties in the state. Our mission is to empower individuals with chronic conditions and disabilities to receive essential care right in the comfort of their own homes and communities. Through the Ohio Home Care Waiver, we offer a comprehensive range of services including skilled nursing care, personal care, homemaking assistance, home-delivered meals, and personal emergency response systems. Additionally, we facilitate accessible home modifications, minor home repairs, specialized medical equipment, out-of-home respite care, adult day care, and supplemental transportation.
