PASSPORT Program

In Fremont, OH, the PASSPORT Program, a vital initiative of the Area Office on Aging, is dedicated to assisting frail and low-income older adults in maintaining their independence at home. By offering a range of consumer-directed service options, we empower seniors aged 60 and over to take charge of their care by hiring their own service providers, including friends, neighbors, and some family members. Our comprehensive services include meal delivery, housekeeping, personal care assistance, transportation, emergency response systems, access to home medical equipment and supplies, adult day services, and minor home modifications. The PASSPORT Program is committed to enhancing the quality of life for seniors eligible for Medicaid's Assisted Living Waiver, ensuring they receive the support they need to thrive in their own living environments.
