Home Delivered Meals

Located in Defiance, OH, Nutrition for Longevity (N4L) is dedicated to providing essential Home Delivered Meals and Medically Tailored Meals to individuals eligible for Medicare Advantage plans, Medicaid Long-Term Services and Supports programs, or the Older Americans Act. Our meal program is thoughtfully designed by a Registered Dietitian to address the specific nutrient needs for improved health, catering to various medical conditions such as Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure, while ensuring meals are free from dairy, peanuts, gluten, and pork. We work closely with Case Managers and local agencies to facilitate meal delivery for beneficiaries, and we also offer a self-pay option for those not covered by insurance, allowing individuals to customize their meal choices at an affordable rate. In addition, N4L provides Medical Nutrition Therapy and Nutrition Counseling, reinforcing our commitment to promoting health and well-being through proper nutrition.
