Ohio Home Care Program

The Ohio Home Care Program, based in Cuyahoga Falls, is dedicated to providing essential assessment and case management services across all 88 counties in Ohio. Our mission is to support individuals with chronic conditions and disabilities in receiving vital waiver services right in the comfort of their homes. We offer a comprehensive range of services, including skilled nursing care, personal care, homemaking assistance, and home-delivered meals, alongside personal emergency response systems and accessible home modifications. Additionally, we provide home maintenance for minor repairs, specialized medical equipment, vehicle modifications, respite care, adult day care, and supplemental transportation. By facilitating home-based care, we strive to enhance the quality of life for our clients, allowing them to remain in their communities rather than transitioning to long-term care facilities or hospitals.
