Ohio Home Care Program

The Ohio Home Care Program, located in Alliance, OH, is dedicated to providing compassionate and comprehensive support for individuals with chronic conditions and disabilities through Assessment and Case Management Services across all 88 counties in Ohio. Our mission is to empower individuals to remain in their homes and communities by facilitating access to a variety of essential waiver services. We offer skilled nursing care, personal care, and homemaking assistance, in addition to vital services such as home-delivered meals, personal emergency response systems, and accessible home modifications. Our program also includes home maintenance for minor repairs, specialized medical equipment, vehicle modifications, out-of-home respite care, adult day care, and supplemental transportation. As part of the Ohio Department of Medicaid’s initiative, we strive to enhance the quality of life for those we serve, ensuring they receive the care they need in the comfort of their own homes.
