Home Delivered Meals

Located in Alliance, OH, Nutrition for Longevity (N4L) is dedicated to enhancing the health and well-being of individuals through our Home Delivered Meals and Medically Tailored Meals programs. We serve eligible clients covered under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, and the Older Americans Act, as well as anyone seeking a self-pay option for nutritious meal delivery. Our meals, crafted by a Registered Dietitian, cater to specific dietary needs and address various medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, while ensuring they are free from dairy, peanuts, gluten, and pork. For those under Medicaid or the Older Americans Act, our services are designed to meet diverse eligibility requirements, and we encourage clients to connect with their Case Managers or local Area Agency on Aging for assistance. Medicare Advantage beneficiaries can also explore meal options as part of their post-discharge care. For individuals not covered by these programs, our self-pay service allows for the flexibility of selecting meals for just $7.75 each, making nutritious options accessible. Additionally, we offer Medical Nutrition Therapy and Nutrition Counseling to further support our clients’ health journeys.
