Cancer Transitions is a supportive program located in New Hyde Park, NY, dedicated to empowering cancer survivors who have completed treatment. While primarily designed for those who have finished their treatment within the last 24 months, our services extend to all survivors at any stage of their journey. We focus on crucial areas of recovery, offering nutritional support, tailored exercise plans, emotional health education, and medical management resources to help individuals regain their strength and thrive after cancer. Our mission is to create a nurturing environment where survivors can find the tools and community they need to navigate life after treatment with confidence.
