Living with Type 2 Diabetes Program

The Living with Type 2 Diabetes Program, located in Pacific, MO, is specifically designed for individuals recently diagnosed with type 2 diabetes, offering a supportive and educational environment free of charge. Over the course of 12 months, participants engage with essential resources, including six e-booklets tailored to various topics relevant to their journey after diagnosis, as well as a monthly e-newsletter featuring seasonal tips and important community events, available in both English and Spanish. To further enhance their learning experience, enrollees who provide their phone numbers gain access to an invite-only monthly Q&A session with diabetes experts from the American Diabetes Association. This program empowers individuals to ask questions about diabetes management and discover the connection between diabetes and heart health, all while receiving valuable giveaways that assist them in thriving with their condition.
