Located in Walled Lake, MI, Cancer Transitions is a compassionate program designed for cancer survivors who have completed their treatment. We focus on individuals who have finished their treatment within the last 24 months, although our resources are beneficial for survivors at any stage of their journey. Our evidence-based approach encompasses vital support services, including nutritional guidance, personalized exercise plans, emotional health education, and medical management support. We are committed to empowering survivors with the tools and knowledge they need to thrive in their post-treatment lives.
