Cancer Transitions is a supportive program located in Clarkston, MI, dedicated to serving survivors of any type of cancer who have recently completed treatment. Designed primarily for those who have finished their treatment within the last 24 months, this evidence-based initiative offers vital resources that can benefit all survivors at any stage of their journey. Participants receive personalized nutritional support, customized exercise plans, and essential education on emotional and medical health management. Our mission is to empower cancer survivors with the tools and knowledge they need to thrive after treatment, fostering hope and resilience within the community.
