The DHHS Community Care Program, part of the Office of Population Health Equity in York, Maine, is dedicated to bridging the gap between individuals and essential health services. With a user-friendly online referral system, we empower Mainers to access a breadth of programs that address health, social, and nutritional needs. Our mission is to serve everyone, especially those facing barriers in accessing vital resources. By submitting a simple referral form, community members can connect with culturally and linguistically appropriate services, support for mothers and children, healthcare navigation assistance, health education, and options for food assistance, heating, and transportation. Additionally, we offer resources for managing stress and anxiety, as well as COVID-related support. We prioritize timely responses, ensuring that a partner organization reaches out within 1-3 days to discuss individual needs and available options.
