The DHHS Community Care Program, part of the Office of Population Health Equity in Standish, ME, is dedicated to bridging the gap between Mainers and essential health and social services. Our online referral system ensures that everyone, particularly those facing barriers to access, can easily connect to a wide spectrum of supportive resources. From culturally and linguistically appropriate services to assistance for mothers, infants, and families, we focus on providing vital support in navigating healthcare, health and wellness education, as well as options for food, heating, and transportation. We also offer resources for managing stress and anxiety and provide information related to COVID-19. By filling out our referral form, individuals will be connected with one of our partner organizations within 1-3 days to discuss their needs and explore available options.
