The DHHS Community Care Program, part of the Office of Population Health Equity in South Portland, ME, is dedicated to breaking down barriers to health and social services for all Mainers. Through our user-friendly online referral system, individuals and families can easily connect with vital resources tailored to their unique needs. We strive to provide culturally and linguistically appropriate services, with specific support for mothers, babies, and children, as well as assistance in navigating healthcare systems. Our program offers access to health and wellness education, options for food assistance, heating, and transportation, and resources for managing stress and anxiety. We prioritize COVID-related support and work collaboratively with local partners to ensure that those who reach out receive timely assistance, usually within one to three days. For personalized support, we invite community members to submit a referral form, as each program comes with specific eligibility requirements that may vary by region.
