The DHHS Community Care Program, part of the Office of Population Health Equity at ME CDC, is dedicated to empowering residents of South Berwick, ME, and beyond by connecting them to essential health programs, social services, and resources they may need. Our inclusive online referral system is designed for everyone, particularly for those who face challenges in accessing vital services. By completing our referral form, you can tap into a wide array of support including culturally and linguistically appropriate services, resources for mothers and children, healthcare navigation assistance, health and wellness education, and options for food, heating, and transportation. We also provide support for managing stress and anxiety, as well as COVID-related resources. Once you submit your referral—one per household—our compassionate team will begin reviewing your needs and typically connect you with a partner organization within 1-3 days.
