The DHHS Community Care Program, part of the Office of Population Health Equity in South Berwick, ME, is dedicated to ensuring that all Mainers have access to vital health programs and social services. Designed with inclusivity in mind, our program assists individuals facing barriers in accessing the resources they are eligible for. By utilizing our easy-to-use online referral system, community members can connect with a broad range of support, including culturally and linguistically appropriate services, comprehensive maternal and child resources, healthcare navigation assistance, health and wellness education, and options for food, heating, and transportation. We also provide support for managing stress and anxiety, alongside resources related to COVID-19. After submitting a referral—one per household—our committed staff will work diligently to match you with one or more partnered organizations who will initiate discussions regarding your specific needs within 1-3 days, ensuring a personalized approach to support.
