The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to connecting residents of Poland, ME, and surrounding areas to essential health programs and social services. Our online referral system provides a streamlined approach for individuals facing barriers to accessing the resources they qualify for. Through our referral form, community members can gain access to a wide array of services, including culturally and linguistically appropriate support, resources for mothers and children, healthcare navigation assistance, health and wellness education, as well as options for food, heating, and transportation. We are committed to offering support for stress and anxiety and providing COVID-related resources. Each referral is handled with care, ensuring a partner organization reaches out within 1-3 days to discuss specific needs and available options, tailored to our community's diverse requirements.
