The DHHS Community Care Program, part of the Office of Population Health Equity at ME CDC, is dedicated to ensuring that every Mainer can easily access vital health programs, social services, and essential resources. Our online referral system simplifies the process for individuals facing barriers in obtaining the support they need. By completing our referral form, community members can connect with a wide array of services, including culturally and linguistically appropriate care, resources for mothers and children, healthcare navigation assistance, health and wellness education, and options for food, heating, and transportation support. We also offer resources for managing stress and anxiety, as well as COVID-related assistance. Once a referral is submitted, typically within 1-3 days, one of our dedicated partners will reach out to discuss specific needs and available options, keeping in mind that each program may have varied eligibility requirements depending on the region.
