The DHHS Community Care Program, part of the Office of Population Health Equity in Limington, ME, is dedicated to helping individuals and families access essential health programs and resources throughout the state. Our online referral system is designed to support Mainers facing barriers to obtaining the services they need. By filling out our referral form, you can connect with a diverse range of resources, including culturally appropriate services, maternal and child health support, healthcare navigation assistance, health and wellness education, and options for food, heating, and transportation. We also provide support for managing stress and anxiety, as well as COVID-related resources. Each household is invited to submit one referral, and our team will work swiftly to connect you with one of our partner organizations, typically within 1-3 days. We are committed to ensuring that every member of our community can access the support they deserve, tailoring our approach to meet the unique needs and eligibility requirements of individuals across our region.
