The DHHS Community Care Program, part of the Office of Population Health Equity in Houlton, ME, is dedicated to helping Mainers access essential health programs and social services seamlessly through our user-friendly online referral system. We welcome everyone, especially those facing challenges in obtaining the resources they need. By completing our referral form, individuals can connect with culturally and linguistically appropriate services as well as support for mothers, babies, and children. We provide assistance navigating the healthcare system, access to health and wellness education, and options for food assistance, heating, and transportation resources. Our program also addresses mental health needs, offering support for stress and anxiety, along with vital COVID-related resources. After submitting a referral, participants can expect to hear from one of our partner organizations within 1-3 days to discuss their specific needs and available options.
