At the DHHS Community Care Program, part of the Office of Population Health Equity in Greene, ME, we are dedicated to bridging gaps for Mainers in need. Our online referral system simplifies the process of connecting individuals with vital health programs, social services, and a spectrum of resources tailored to those facing challenges in accessing needed assistance. This inclusive program offers a wide range of support—from culturally and linguistically tailored services to resources for mothers, infants, and children, as well as navigation assistance through the healthcare landscape. We prioritize health and wellness education, providing access to food, heating, and transportation options, along with vital support for stress and anxiety, and COVID-related resources. By completing a single referral form per household, individuals can begin their journey toward well-being, with our dedicated staff coordinating outreach from appropriate partner organizations within 1-3 days. We understand that eligibility criteria may vary and that resources differ by region, ensuring a tailored approach for every community member.
