The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to helping residents in Gorham, ME, access vital health and social services through an easy-to-use online referral system. By filling out our referral form, you can connect with a wide range of support options, including culturally appropriate services, assistance for mothers and children, guidance in navigating the healthcare system, health and wellness education, and resources for food, heating, and transportation. We also provide support for managing stress and anxiety, as well as COVID-related resources. Once a referral is submitted, our dedicated staff will connect you with one of our partner organizations within 1-3 days to start discussing your needs and exploring available options.
