The DHHS Community Care Program, part of the Office of Population Health Equity in Gorham, ME, is dedicated to empowering residents by streamlining access to essential health programs and social services throughout the state. Our online referral system helps individuals and families overcome barriers to obtaining vital resources they qualify for, ensuring that support is available for everyone in need. By simply filling out our referral form, community members can tap into a wide array of services, including culturally and linguistically appropriate assistance, resources for mothers and children, guidance through the healthcare system, health and wellness education, as well as access to food, heating, and transportation assistance. We also offer support for managing stress and anxiety, along with COVID-related resources. Typically, within 1-3 days, one of our partner organizations will reach out to discuss specific needs and options tailored to each individual.
