The DHHS Community Care Program, part of the Office of Population Health Equity in Gorham, ME, is dedicated to bridging gaps in access to vital health programs and social services for all Mainers. By utilizing an efficient online referral system, we help individuals and families navigate a variety of resources tailored to their unique circumstances. Our services encompass culturally and linguistically appropriate support, maternal and child health resources, guidance through the healthcare system, health and wellness education, and options for food assistance, heating aid, and transportation. We also address mental wellness by providing resources for stress and anxiety, alongside COVID-related assistance. Each household is invited to fill out a referral form, ensuring that we can connect them with the right support within 1 to 3 days, as we collaborate with local partners to address specific needs based on eligibility and regional availability.
