The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to helping residents of Farmington, ME, access vital health programs and social services through an easy-to-use online referral system. Our mission is to remove barriers to essential resources for all Mainers, especially those facing challenges in navigating available services. By filling out our simple referral form, individuals can connect with support for a wide range of needs, including culturally and linguistically appropriate services, assistance for mothers and children, health and wellness education, and resources for food, heating, and transportation. Additionally, we offer support for stress and anxiety and provide COVID-related resources. Once a referral is submitted, our compassionate team will reach out within 1-3 days to discuss specific needs and connect individuals with the appropriate organizations, ensuring that everyone receives the support they deserve. Each program has unique eligibility criteria, and availability may vary by region, but our commitment to equitable access remains steadfast.
