The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to providing easy access to essential health programs and social services for residents of Fairfield and throughout Maine. Through a user-friendly online referral system, we connect individuals and families to a wide range of resources designed to overcome barriers in accessing the support they need. From culturally and linguistically appropriate services to resources for mothers, babies, and children, we ensure that all Mainers can find assistance with healthcare navigation, health and wellness education, food assistance, heating and transportation resources, and support for managing stress and anxiety.
